(Port Louis, Mauritius) - The Apollo Bramwell Hospital, the hi-tech tertiary care hospital based at Moka, will soon set up an advanced department to diagnose and treat cancer.

 
 
  Discharge Planning                
 

Discharge Planning is a powerful value-added service for patients, physicians and families at Apollo Bramwell Hospital (ABH). Discharge Planning serves several objectives for great patient care. From the moment a patient is admitted to our hospital, a multi-disciplinary team of healthcare professionals led by the admitting physician and coordinated by a speciallytrained nurse begin planning the care the patient will receive after discharge from the hospital. Based upon the admitting diagnoses, hospital services received, and the discharge summary, follow-on care is planned in a manner that ensures appropriate Continuity of Care.

The Discharge Plan ensures a post-discharge consultation with the patients’ physician is scheduled, that post-discharge medications are prescribed and delivered, and that essential pharmacist counseling has occurred. This is a level of discharge coordination found in many good hospitals. At ABH, we take Discharge Planning to new levels based upon our Centers of Excellence, our focus on maintaining a Continuum of Care, and our dedication to superior clinical outcomes.

If the discharge diagnosis suggests that diet planning is appropriate, that the patient would benefit from a regimen of physical therapy, rehabilitation, or fitness training, that a broader Wellness Assessment is indicated or that another specialist referral is required, it is not only noted in the Discharge Plan, but in addition, ABH staff will make the required appointments or initiate contacts on behalf of the patient and family.

ABH Discharge Planning extends beyond the needs of the patient to include support, education and orientation to family members. ABH Behavioral Health Services provides the services of a Physiatrist and a Psychologist (Counselor) to support the Discharge Planning process.

This can include one-time or recurring counseling as required. The hospital maintains a number of staff who function as clinical educators to support the needs of patients and family to understand the clinical conditions impacting them.
